Types of Weight Loss Surgery: Lap Band

Restrictive operations serve only to restrict food intake and do not interfere with the normal digestive process. To perform the surgery, doctors create a small pouch at the top of the stomach where food enters from the esophagus. Initially, the pouch holds about one ounce of food and later expands to hold more. The lower outlet of the pouch has a small opening to delay the emptying of food from the pouch and cause a feeling of fullness.

Restrictive operations for obesity include the Adjustable Gastric Ban (LAP-BAND), which restricts the size of the stomach using a band.

Although restrictive operations lead to weight loss in almost all patients, they are less successful than malabsorptive operations in achieving substantial, long-term weight loss. Some patients regain weight. Others are unable to adjust their eating habits and fail to lose the desired weight. Successful results depend on the patient's willingness to adopt a long-term plan of healthy eating and regular physical activity.

Gastric Band Support Group Recap - August

The Clarian Bariatrics' support group meeting for gastric band patients was held on Tuesday, August 18. Did you know that Clarian Bariatrics offers monthly support group for gastric lap band weight loss surgery patients on the third Tuesday of each month from 6 p.m. to 7p.m.? Please consider joining us at the next session.

The topic this month was "Why We Overeat?"

A few of the reasons we overeat are steeped misinformation and myth, but there are many with theories with scientific data to back it up. Some of the myths and truths include: 
  • Emotional eating, which could include feeling like there is no hope or giving up.
  • Foods affecting neurotransmitters in the brain including sensory cues like vision, smell and taste.
  • Personal and family history and traditions.
  • The ability to get food 24/7.
  • Making our life "chaotic" through various commitments.
  • Even the theory that the food and restaurant industry are manipulating fat, sugar, and salt content.

However, this is a very small list of reasons. Solutions to combat why we over eat include:
  • Learning fact from myth.
  • Creating structure in your life.
  • Practice eating right by picking foods that satisfy not stimulate.
  • Anticipate events in your life and seize control.
  • Stop destructive thoughts as they happen not afterwards.
  • Listen to yourself!
The group concluded that there is not one problem or solution to overeating. Ultimately, it is about making choices and understanding the benefits and consequence of those choices.

What's Coming Up This Week at Clarian Bariatrics?

Support Groups
We will be having the following support groups this week!

  •  Clarian Health Cicero located at 1160 South Peru St., SR 19 in Cicero. Support group is held on the second Thursday of the month from 6 - 7 p.m.
  •   Pre-Surgery Coping Skills Group held each Thursday at the Clarian Bariatrics clinic from 4 p.m. - 5:15 p.m. This group is designed to explore changing family and friend relationships and social situations, including how to survive eating in social situations, how to respond to people who "push" food on you, and how to cope with people's reactions to your new weight loss and lifestyle. This group is free, but we ask that you please call 317-275-7010 or 877-275-2555 prior to attending.
Exercise Classes
We will be holding the following exercise classes this week!

"Walk the Talk" Walking Group - Join us weekly for a walking program and support group that will help you burn calories, make friends and keep you on track with your goals.  We meet every Friday from 11:30 a.m. - 12:30 p.m. at the Clarian Bariatrics outpatient clinic.  At these group we will walk, talk and get fit together!  Feel free to bring a sack lunch and chat with us during a support group meeting after the walk.  Make sure you bring a water bottle and wear your good tennis shoes!  In addition to our weekly walks, we participate as a team in charity walks related to disease prevention and research.  There is a small cost for to sign-up for the walks, but the experience is priceless. If you cannot make it to Clarian Bariatrics every week to walk with us, feel free to keep up with a walking program and join us for the charity walks as often as you can!

Clarian Bariatrics Performs First Single Incision Lap-Band Procedure in Indiana

Don Selzer, MD, a physician with Clarian Bariatrics, is the first in Indiana to perform a single incision LAP-BAND® System surgery, which results in reduced scarring, less post-operative pain and a recovery time three to four days less than standard adjustable gastric banding procedures. 

This procedure utilizes single incision laparoscopy, a minimally invasive approach in which the surgeon operates through a single entry point, typically the patient's belly button. All of the instruments needed for the procedure are used through this single incision and, after surgery, the scar is almost completely hidden inside the belly button making the procedure virtually scarless.

Since the first procedure on March 3 at Clarian North Medical Center, Selzer has completed two additional procedures and joins an elite group of physicians from Missouri, New York, Texas, Michigan and Florida where similar surgeries have been performed. Selzer will now begin performing this type of surgery on a weekly basis.

“Since the first gastric banding procedure in 1992, weight loss surgery has come incredibly far,” Selzer, an Assistant Professor of Surgery with the Indiana University School of Medicine, said. “The single port procedure is just another example of how surgeons are constantly looking at innovative ways to improve patient outcomes and recovery times.”

The LAP-BAND® Adjustable Gastric Banding System, manufactured by Allergan Inc., is a silicone band that is surgically placed to reduce the amount of food a patient can hold in his or her stomach. The band helps patients control food intake and supports long term, sustainable weight loss. Although this procedure is performed laparoscopically, it typically requires at least five abdominal incisions.

The single port option helps meet the growing demand of individuals wishing to experience sustainable weight loss through the use of bariatric surgery but who desire a quick recovery. The number of weight-loss surgery procedures performed annually has surged over the last several years. According to a report released in 2007 by the Agency for Healthcare Research and Quality, the number of bariatric surgeries has grown from 16,000 procedures performed in 1992 to 220,000 now performed annually.

Clarian Bariatrics, a designated Center of Excellence by the American Society of Metabolic and Bariatric Surgery, offers patients three types of weight loss surgery through its innovative program. Last year, Clarian Bariatrics surgeons completed nearly 400 procedures and the comprehensive program boasts some of the best clinical outcomes of any other bariatric surgery program in the United States.

News article: Obesity Reduces Women’s Brain Power

Not a good week for women's health and obesity. The following article came across my Google alert.

Obesity may be linked to a reduced volume of gray matter in the brains of postmenopausal women, according to a new study published in the journal Psychosomatic Medicine: Journal of Biobehavioral Medicine.

“A subset of women from the Pittsburgh Healthy Women Study (HWS) who had been followed for an average of 15 years was invited to participate in brain-imaging studies,” said lead author Isabella Soreca, MD, an assistant professor at the University of Pittsburgh. “We found that those who had gained the most weight had lower gray matter volume. Women who gain weight tend to have high blood pressure, impaired glucose tolerance and other health concerns that are known to affect the brain.”

The following factors will be considered in determining if you are a candidate for bariatric surgery:
  • You have tried to lose weight by changing your eating habits and exercise and are still severely obese.
  • You are unable to perform routine daily activities without being seriously affected.
  • You understand the procedure and the lifestyle changes you will need to make after bariatric surgery.
  • You are motivated to make a lifelong behavioral commitment that includes well-balanced meals and physical exercise.
  • You understand that this surgery won't solve all your problems, but can make you healthier -- and probably much happier.
Before patients undergo surgery for morbid obesity, they must qualify for our bariatrics program. Our basic program guidelines are:
  • Being 100 pounds or more over your "ideal body weight," according to the established weight charts.
  • Having a Body Mass Index (BMI) greater than 40 with no co-morbidities or greater than 35 with serious co-morbidities.
  • Being able and willing to make the necessary lifestyle changes.

Bariatric surgery is now accepted as the best and most effective treatment for morbid obesity. About 65 percent of all bariatric surgery patients are able to lose their excess weight and keep it off for more than five years.

When you consider that only approximately two percent of nonsurgical treatment patients are able to lose and keep off their weight for more than a year, you can appreciate these surgical results.

Health problems resolved or improved after surgery:
  •     86 percent diabetes
  •     70 percent high cholesterol
  •     78 percent high blood pressure
  •     84 percent sleep apnea

Expected weight loss (weight loss occurs for 18-24 months after surgery — mostly in first year):
  •     Lap-band surgery — 40-48 percent excess weight loss (national avg. = 47 percent)
  •     Gastric bypass surgery — 65-80 percent excess weight loss (national avg. = 62 percent)
  •     Duodenal switch — excess weight loss (national avg. = 70 percent)

May Support Group Recap, Part I

The Clarian Bariatrics support group meeting for gastric lap band patients was held on Wednesday, May 20.  This month's topic was "Developing a Positive Self Message."

Most self talk is negative. Negative self-talk may cause you to doubt yourself, create uncertainty, and perpetuate poor self esteem and poor self confidence. Since self-talk is mostly negative, this will work against you in the long run.

Change your self talk to positive self message. Changing to positive self-talk will give you a new message to help you overcome long-time fears and obstacles. You will become more confident around others and feel more in control. All you have to do is learn to talk to yourself and with a positive self message!

Create awareness of what and how you talk to yourself. Write down when you are self critical, negative, and judgmental. This will increase awareness when you becoming demeaning to yourself. Replace negative thoughts with positive and flexible messages.
Focus on what you can control!

Write down all negative self-messages and reword to make into positive self-message. For example - Negative self-message – “I will never be able to lose weight and keep the weight off.” Positive self-message – “I will take one day at a time and overcome difficulties the best that I can.”

I am currently at the office packing up my gear to take out to the Indianapolis Motor Speedway and I am so excited! I think that this former patient event is going to be great, and I am even more thrilled that the weather will (hopefully) cooperate. Hypertension (high blood pressure), one of the many common conditions associated with obesity, refers to the pressure that blood applies to the inner walls of the arteries.

It estimated that excess body weight (including overweight and obesity), accounted for approximately 26 percent of cases of hypertension in men and 28 percent in women, and for approximately 23 percent of cases of coronary heart disease in men and 15 percent in women. Obese individuals have an increase in fatty tissue that increases their vascular resistance and in turn increases the work the heart has to do to pump blood throughout the body.

The Bariatric Valentine, continued

Due to today’s economy, many of us realize that we are unable spend a lot of cash to throw a grand Valentine’s Day for our loved ones.  Most restaurants and retailers raise the prices of items in high demand, such as chocolates, restaurant meals and cards.  Luxurious presents and fine dining are always nice, but there are ways to celebrate Valentines Day without breaking your budget or your waistline. 
  • Take a professional dance class – Local ballrooms are offering reduced rates for private and group sessions.  Once you learn the steps, you can practice together anytime.  This will connect you as a couple, and it’s a fun way to enjoy each other’s company. 
  • Does he love a clean car – Arrange to have his car detailed with an indoor polish and an exterior shine.
  • Sushi lessons or couples cooking classes- Many local chefs teach private classes for less than $100 per session. 
  • Paintball game – Paintball is a great day to have fun and exercise at the same time.  For about $30 per person you can play all day. Think of the fun you’ll have shooting your husband with paint. 
  • Trip to the spa/couples massage – nothing is more relaxing than a massage with your loved one.

January Gastric Bypass Support Group Recap

The Clarian Bariatric's  support group meeting for gastric lap band patients was on Wednesday, January 21.  This month's topic was protein. We discussed why it was a good food source, an explanation of why protein is important, the best protein source in supplements, how much do you need and can you eat too much protein.

Proteins in our diet are made up of building blocks called amino acids. There are 20 amino acids.  Ten are “essential” meaning they cannot be made by the body and must come from our diet. There also 10 non-essential amino acids,  which can be created by the body.

These amino acids are used by the body to make specific proteins such as enzymes, hormones, skin, hair, muscles and transport protein, etc. Every function in the body requires some type of protein. Animal based foods are the best food source and are called “Good Quality Protein” because they provide all essential amino acids.

Plant based foods typically lack one or more essential amino acids therefore are not considered good quality protein. Therefore, the best way to obtain protein from diet are from all types of meat, cheese (2 percent, low fat or fat free) or egg and adding in plant based foods.
